This page explains what this website does with the details you send us. Switchback Physical Therapy is a physical therapy clinic in Winooski, Vermont.

What we ask for

The contact form asks for your name, email, phone number, how and when you would like us to reach you, and a short message. Please do not put medical details in that message. We will ask about your health at your visit, in person or on our secure patient portal.

What we do with it

We use what you send to reply to you and to schedule a visit. We keep your records private and share them only as the law allows. We do not sell your details.

What this site does not do

This site does not use advertising pixels, session replay or embedded schedulers. The online booking button opens our scheduling system in a new window, and that system has its own terms.

Your choices

You can ask us what we hold about you, ask us to correct it, or ask us to stop contacting you. Call the front desk at (802) 555-0107 or write to hello@switchbackpt.com.

Changes

If we change this page, the date at the foot of it changes too.
